The building was erected towards the end of the 1300s. Tradition goes that two inhabitants of the ancient Chiusini village decided on its construction. The Madonna appeared in a dream to a woman, indicating the place where the church should be built. In that area, the woman and her husband found a blossoming sage branch in the middle of winter.
Composed of one room in Gothic style, the building is a unique example of its kind due to its characteristic bell tower.
